Passport Services are by appointment only. Book your appointment.

As a passport acceptance agency, we process applications for initial passports and minor renewals only. If you need an adult renewal, this is accomplished via the U.S. Mail. Check here for processing times.

There is an additional U.S. Government fee to expedite your application.

Important Information

The federal government has extended the deadline for REAL ID to May 7, 2025. As of that date, standard state-issued driver’s licenses and identification cards will no longer be accepted to board domestic flights within the United States, or when entering certain federal facilities. Passport Appointment Checklist

All applications require a new passport photo. We offer photo services for $15. We recommend wearing a dark shirt or jacket. Light colors do not work well with our equipment.

Please note: The U.S. Department of State does not allow you to wear logos or camouflage in your photograph.

All applications require a check or money order;  the U.S. Department of State will only accept these forms of payment.

Important: If you’re submitting applications for more than one individual, a separate check or money order is required for each applicant.

Please note: Payments to the Clerk’s Office – for services such as photo processing, application handling fees, and copying services – must be made separately from payments to the U.S. Department of State.

First-Time Passport

  • DS-11 application
  • Original birth or naturalization certificate + one (1) copy
  • Both parents’ photo IDs (copied front & back)
    • If one parent can’t attend: bring a notarized DS-3053 form + copy of the ID used for notarization
  • Checkbook or money order

Renewal

  • DS-11 application
  • Current passport
  • Birth certificate copy
  • Both parents’ photo IDs (copied front & back)
    • Same consent requirements as above
  • Checkbook or money order

First-Time or Renewal

  • DS-11 application
  • Original birth certificate + one (1) copy (for first-time applicants) or current passport + one (1) copy of birth certificate (for renewal)
  • Photo ID (copied front & back)
  • One parent’s photo ID (copied front & back)
    • If parent can’t attend:
      • Implied consent (e.g. check with parent's name) or
      • Notarized DS-3053 + copy of the ID used for notarization

Fees:

There are fees that go to the U.S. Department of State and fees to the Township.
 

  1. Check Payment (Personal, Cashier’s Check, or Money Order) must be brought to your Passport appointment. Made out to the U.S. Department of State.

2.Township service fees can be paid by cash, check, money order or credit/debit card (a convenience fee applies to credit/debit cards)

Notary service for Township residents:

For residents and non-residents there is no charge for a single copy of the signature page and notarized page (if different). Any additional copies will be charged at $1.00 per page.
