While the Senior Center is closed to the public these are the transportation services that are currently available.
Medical Transportation Services
Medical Transportation is available to residents of Bloomfield Township and Bloomfield Hills (limited availability) through SMART funding. Application for program is required. Rides originate in these communities and will regularly provide transportation within a 10 mile radius; fees may be incurred for rides greater than 10 miles from current residence. There is a 45% co-pay charged monthly to eligible users. If the shared co-pay cost presents financial hardship, please inquire about application for co-pay waiver.
Residents must be 60 years of age or older or 50 and over with handicap. (Written confirmation from licensed physician is required.) The limited amount of funds and high cost of medical transportation mandates that we implement qualifying guidelines for receiving services. Transportation clients are responsible for 45% co-pay to BTSS for all rides. If this co-payment presents a financial hardship for you, please contact Senior Services at 248-723-3500 about application for co-payment waiver.
A completed application form must be on file for all individuals prior to receiving medical transportation services. Proof of residency must accompany the application form. Transportation co-pay waiver will require proof of income as well.
For information please call 248-723-3500.
